With only four reported problems (a record low) after the upgrade, the new Epic Coordinated Care Management (CoCM) module went live for 36 LA ambulatory pilot users on August 10, 2020. The CoCM module is comprised of a set of tools that streamline patient care documentation for care manager RNs, social workers, and care coordinators.
The CoCM platform gives caregivers a fuller picture of the social, behavioral, and clinical factors for each patient all in one place, enhancing capture of key information as coded data elements, as opposed to note-based/non-reportable information. This complete picture helps clinicians make important decisions about how to improve the overall health and quality of life for each individual, increasing focus on social determinants of health.
“It is not only about tracking typical health indicators, such as blood pressure, smoking history, and weight. It also supports a more whole-person approach, focusing on the patient’s priorities and the ‘life factors‘ that might be getting in their way,” said Dr. Karen Boudreau, SVP, Enterprise Care Management. “This new upgrade allows the system to be much more robust than before.”
The CoCM platform enables better coordination across services, so all caregivers can take a collaborative approach to help patients achieve their goals.
While this upgrade was completed remotely, the project was a success. The project team provided two weeks of real-time support after go-live, using virtual tools like Teams Meetings. Users could join the meetings to report issues, and share questions and concerns to get a response quickly. Other team members were able to learn from questions already asked and handled through Teams chat.
On October 26, 2020, a full enterprise roll-out of CoCM for all ambulatory Care Management team members will begin, as part of the October Epic Upgrade effort. We expect a similarly smooth transition, relying heavily on Super Users and Teams functionality in each location. A significant factor in the success of the project has been the high degree of collaboration between the project team, the Ambulatory Care Management Standardization workgroup led by Sheila Joyce, the Tapestry/Healthy Planet project team, and the managed care teams in LA and Orange County, led by Joyce Komori and Cammie Flores.
